You or your loved ones may be returning home from recent hospital stay or simply just unable to do some tasks as previously can. HomeCare Perks is here to provide you additional support to continue to recover and prevent unexpected return to the hospital. HomeCare Perks totally understands how challenging it is to ensure safety of someone who lives alone but still in recovery stage. Our Registered Nurse will create an individualized plan of care and services designed to assist you achieve your goal. Our personal care service entails an over-all coordination and development of patient’s plan of care, which includes comprehensive health and psycho-social evaluation, monitoring of the patient’s condition, health promotion and prevention coordination of services, teaching and training activities, and direct nursing care. Our Registered Nurses will continue to closely monitor our Certified Home Health Aides to ensure that all your needs are met. We do not require any long-term contracts or commitments.
